Step 1 - Information you will need before you start
Please read the following information and ensure that you have all the necessary documents before proceeding to the online form.
Information required:

if you have had previous dealings with OSR for any other revenue type then use your existing client ID
your business name
address details
contact name and daytime telephone number
when the business first started to employ
if you are a company you will need to provide us with your ABN
the date you first employed staff in NSW or Interstate
current and past four years worth of NSW and/or interstate wage details including superannuation and FBT
details of other group member wages.
Grouping
list the names of all group members who pay wages and directors' fees in NSW and/or Interstate
Banking Details
If you want to pay by direct credit, please provide your:
BSB number
account number
account name
financial institution name
branch

If you have all the necessary documents, it should take you approximately 20 minutes to complete and submit this form online.
Warning:
The current version of our online form does not allow you to save your work and come back at a later time to complete the registration.
If you commence filling out the form and then decide to abandon it, you cannot save your work and will need to start again.
