You can use the change of details form to:
cancel your registration
update your contact details
update address details
change your business name
change your group structure.
Read the following information and ensure that you have all the necessary details before starting the online form.
Required information
-
your client ID and correspondence ID from any recent OSR correspondence, or your AUSkey. For more information on AUSkey go to:
new contact name and daytime number
new correspondence address
new business address
if you are cancelling your registration you will need to provide:
the date you ceased to employ staff in NSW and/or interstate
the reason for cancellation
NSW and/or interstate wages paid up until you ceased to employ staff in NSW and/or interstate.
if your grouping has changed you will need to provide:
other group member names and Client IDs
the date when the change in grouping occurred
the reason for the change.

If you have all the required information, it should take you about 15 minutes to complete and submit this form online.
Warning
