your existing Client ID and Correspondence ID from any recent OSR correspondence or your AUSkey
number of full time equivalent (FTE) employees at the anniversary date of the job/s that you are claiming.
Employee’s name, if the current employee has replaced the initial employee in that job
Bank Account details if not previously entered
Estimated time
If you have all the necessary documents, it should take you approximately 10 minutes to complete and submit this form online.
