Step 1 - Information you will need before you start

Please read the following information and ensure that you have all the necessary documents before proceeding to the online form.

Information required:

• If you have had previous dealings with OSR for any other revenue type then you will need to provide us with your existing client ID

• You will need your company name

• Address details

• Contact name and number

• You will need your client name

• The date you first became liable for:

• Health insurance levy or

• State ambulance insurance plan

• If you are a company you will need to provide us with your ABN

Banking Details

If your preference is for direct credit you will need to provide your:

• BSB number

• Account number

• Financial institution

• Branch

• Account name

Estimated time:

If you have all the necessary documents, it should take you approximately 15 minutes to complete and submit this form online.

Warnings:

The current version of our online form does not allow you to save your work and come back at a later time to complete the registration.

If you commence filling out the form and then decide to abandon it, you cannot save your work and will need to start again.
