This form has changed. The new login requires you to enter your Client ID and Correspondence ID which are shown in your health insurance levy booklet.

Information you will need before you start

Please read the following information and ensure that you have all the necessary documents before proceeding to the online form.

Getting started:

• You will need your Client ID and Correspondence ID from your health insurance levy booklet.

Information required after login:

• change of name (you will need to forward the certificate of change of business name to OSR)

• if you are a company you will need to provide us with your ABN / ACN

• new contact name and daytime number

• new correspondence address

• new business address

• if you are cancelling your registration you will need to provide us with the effective date and the reason for the cancellation of your registration

Estimated time:

If you have all the necessary documents, it should take you approximately 15 minutes to complete and submit this form online.

Warning:

The current version of our online form does not allow you to save your work and come back at a later time to complete the registration.

If you commence filling out the form and then decide to abandon it, you cannot save your work and will need to start again.
