Details you need to provide to us
Any changes to your organisation’s name, since the exemption was approved. If the name has changed, you will need to provide the new name of the organisation.
Any changes to the organisation’s governing rules (specifically the organisation’s objects and/or non-profit status) since the exemption was approved.
These include the organisation's:
- articles of association.
If the governing rules have changed, you will need to send us a letter detailing the changes and supply a copy of your most recent governing rules.
If you have all the required information, it should take about 5 minutes to complete and submit this form.
Information collected from you on this form is required by the Office of State Revenue (OSR) to determine if you have a liability or entitlement. The information may be provided to third parties with your consent or as required or permitted by law. OSR will correct or update your personal information at your request.
